Upload Your Childcare Application Packet

Thank you for choosing the Titusville YMCA! To begin the enrollment process, please use the registration link below to upload your completed application packet.

Important Notes:

• You will be prompted to sign in to complete the upload. If you’re unsure of your login credentials, please do not create a new account. Instead, choose to “continue without an account” or contact us at 814-775-0357 for assistance.
• Physical Signatures: Some sections of the application packet require physical signatures. If you’re unable to provide these signatures at the time of upload, don’t worry! We’ll review your packet and reach out to you to collect the necessary signatures later, if needed.
• Child Health Form: Your child’s doctor must complete the health form included in the application packet. The completed form can be faxed to us directly by the doctor’s office, or you can bring it in with your child.
• What’s Next: Once you submit your completed packet, our Child Care Director will review it and contact you if any information is missing. Once everything is in order, you will receive a call to confirm your child’s acceptance into the program. After confirmation, we’ll finalize your child’s enrollment, and you’ll be guided through setting up payment options.
